Prior-Auth Paperwork Agent

Drafts the clinical-justification narrative and tracks follow-ups around your CoverMyMeds / Surescripts ePA, inside a BAA-covered tool only.

The real prior-auth workflow at most pharmacies already runs through electronic prior authorization: CoverMyMeds or Surescripts ePA, both free to pharmacies, pull the right payer form, pre-fill the prescription fields from the claim rejection, and route the request to the prescriber and payer electronically. If you're still faxing paper forms for payers that support ePA, fix that first — it's free and it's the bigger win. This agent does not replace ePA. It covers the two things ePA leaves to you: writing the clinical-justification narrative (turning the prescriber's stated reasons and prior-therapy history into the clear, payer-specific wording that the free-text field needs) and tracking follow-ups so a pended request doesn't sit silently past its window. Because a PA request is inherently full of real clinical information, this agent runs only inside a confirmed BAA-covered AI environment, and it never invents or guesses justification on its own — it drafts only from what the prescriber's office actually provided, flagging any missing element instead of filling the gap.

What this agent takes off your plate

  • A clear, payer-specific clinical-justification narrative drafted in minutes from the prescriber's notes, ready to paste into the ePA free-text field after pharmacist review
  • ePA (CoverMyMeds / Surescripts) stays the system of record for the form and submission — the agent never duplicates or replaces it
  • Missing required fields flagged before submission instead of discovered as a rejection two weeks later
  • Consistent, payer-specific formatting that matches what each plan actually requires
  • Runs only inside a confirmed BAA-covered environment — never a free consumer AI tool
  • A tracking log of PA status so nothing silently expires while awaiting a decision

Real costs & ROI

  • Claude or ChatGPT with a signed BAA (Enterprise plan or API build)Enterprise / API pricing — confirm
As of writing, neither vendor includes a BAA on Pro/Team/Business seats — a BAA is available on Enterprise plans or via an API-based build. Expect materially higher cost than the consumer tiers and get the BAA in writing before any real data is entered.
  • CoverMyMeds or Surescripts ePA$0/mo
free to pharmacies; usually already integrated with your PMS. Payer-specific question sets come through the ePA, so you only need a saved form library for the payers that still require fax
  • Secure fax or payer portal submission tool$0–20/mo
many pharmacies already have this; a HIPAA-compliant e-fax service runs roughly $10–20/mo if you need to add one

The math

Time your own PA process before you build this. ePA already handles the form-pulling and field transcription; what remains is the part a tech or pharmacist does by hand — reading the prescriber's notes, writing a justification that answers the payer's specific question set, and chasing pended requests. If that runs 10–15 minutes per PA and you handle 15–25 a week, that's roughly 3–6 hours a week at a blended $30/hour tech rate. Cutting the drafting half of that time in half is a realistic target once the prompt is tuned to your top payers, and you can measure it against your own baseline in the tracking log. The secondary win is fewer pended-then-expired requests, which otherwise cost another full cycle of delay for the patient waiting on the medication. Compare that against your actual BAA-covered AI pricing, not a consumer-seat price.

Build steps

1

Confirm ePA is live, then build your justification-narrative templates on a fictional sample

First, confirm your pharmacy is actually using CoverMyMeds or Surescripts ePA for every payer that supports it — check your PMS's PA integration settings, and if a rejected claim doesn't already offer a "start ePA" button, get that switched on before anything else. Then, before any real patient data is involved, collect the question sets your top payers ask inside the ePA (the free-text justification prompts, step-therapy questions, and criteria checklists) and test the narrative-drafting logic on a made-up, fictional sample. This step alone is worth doing — it forces you to see exactly what each payer wants argued, not just filled in.

Data you need

Pull the question sets from your top 5–10 payers as they appear in CoverMyMeds / Surescripts, plus the blank fax forms for any payer that still requires one (these are public forms, not patient data). Create a fictional sample prescription and clinical justification for testing — do not use a real patient's information.

Prompt
I want to build clinical-justification narrative templates for prior authorizations at my pharmacy. We submit through electronic prior auth (CoverMyMeds / Surescripts ePA), which handles the form and prescription fields — what I need help with is the free-text justification narrative and the payer-specific criteria questions. This is a fabricated example for template design only — no real patient data.

My top payers and the questions their ePA asks for this drug class (paste the question set or describe): [PASTE OR DESCRIBE 3–5 PAYERS]

Fictional sample PA request for testing:
- Medication (made-up): [DRUG NAME, STRENGTH, QUANTITY]
- Payer: [PAYER NAME]
- Fictional clinical justification provided by prescriber's office: [MADE-UP JUSTIFICATION TEXT]
- Fictional prior-therapy history provided: [MADE-UP]

Please:
1. Build a **standardized internal intake** that captures everything any of my top payers might ask (diagnosis as stated by prescriber, prior therapies tried and outcomes, contraindications to formulary alternatives, duration, monitoring plan) so a tech collects it once from the prescriber's office
2. **Map that intake to [PAYER NAME]'s specific ePA questions**, showing which intake item answers which question
3. Flag **which elements are commonly required but easy to miss** across payers (dates of prior trials, ICD-10 codes if provided by the prescriber, step-therapy documentation, quantity-limit exception rationale)
4. Draft the **justification narrative** using only the fictional sample data, in the wording and length [PAYER NAME]'s free-text field expects
5. Draft a **fax-form version** for any payer on my list that does not support ePA

These templates are what the real-data prompt in Step 3 will reuse.
2

Confirm your BAA before entering a single real PA request

This is the mandatory compliance checkpoint. Confirm a signed Business Associate Agreement is active with your AI vendor and get sign-off from your pharmacist-in-charge or compliance contact before any real PA data is entered.

Data you need

Contact your AI vendor's sales or compliance team and get written confirmation of BAA coverage on your specific plan. Loop in your pharmacist-in-charge to approve the workflow. Do not skip this because "it's just a form" — a PA request contains real diagnosis and medication information, which is squarely PHI.
